Appointment Booking
Please note down the time of your appointment as well as the location, as we have more than one set of rooms.
Parking and Public Transport
Once your appointment has been booked you will receive an email about parking and public transport options relating to the location you have chosen to see Dr Levin at.
What to Bring
Please bring your referral, Medicare Card, Private Health Insurance details and any X-rays / CT / MRI scans, hearing tests or allergy test results.
If you do not bring a referral you will not be eligible to receive the rebate from Medicare for both the consultation and any additional procedures performed.

You will receive an SMS reminder 1 or 2 days prior to your appointment. If you need to reschedule or cancel your appointment, please contact us.
